We want your visit to be as stress-free as possible. Our staff will do everything we can to make sure your experience is pleasant and convenient. If you have any questions about these forms, please don’t hesitate to contact us.

What to Expect

When you visit our office, you will be warmly welcomed and we can answer any questions you may have about completing the paperwork so that Vision One Eyecare may see you. Our team will work together to evaluate your eyes and provide relevant eye care treatment information, review your vision benefits and payment options, fit you for glasses or contact lenses, and focus on exceeding all of your expectations. If at any time you have questions about the process or your specific health information, please ask.

Patient Forms

For your convenience, we have made our patient forms available here online so that you can fill them out at your leisure.
When you arrive for your appointment, please bring the completed forms with you and give them to the receptionist upon check-in.

If you prefer to fill out the forms at our office, we encourage you to arrive for your appointment a few minutes early.

Insurance Accepted

Our office bills insurance companies for office visits and eye examinations as a courtesy to our patients. 

All co-payments, co-insurances, and out-of-pocket charges are collected at check-out for each office visit and cannot be waived due to our contract with your insurance company. It is the responsibility of each patient to know their insurance company and benefit coverage. If we are unable to verify your insurance, you will be financially responsible for services performed that day and we will provided you with an itemized fee bill so you can file a claim for reimbursement from your insurance. Vision One Eyecare accepts all the following form of payment: cash, credit cards, and debit cards.
